If someone has a severe or aggressive bunion or swelling linked to to circulation like venous insufficiency, kidney disease, or congestive heart failure where there is a pooling of fluid in the legs, ankles and feet, then an extra wide shoe may be required to get the additional volume into the shoe to accommodate the more severe swelling. The extra wide width is the most common width for someone with diabetes along with the next width up, the 2X-Wide (4E) width shoes.
